Call 111 - When it is less urgent than 999

New Patients

To register as a new patient please call at the reception desk and request a new patient registration form. After completion return it to the reception desk and please allow 48 hours for approval. PLEASE NOTE you will also be asked to produce photo ID (passport, drivers licence etc.) and proof of address to validate your registration. 

After registration is approved you will be asked to make an appointment with a nurse for a new patient check. This entails a brief medical history and BP check.

From the 5th January 2015 we are able to offer to patients out of our natural catchment area the opportunity to register with us as an 'Out Of Area' patient which means they can still attend the practice for appointments etc but are not eligible for home visits. Ask at reception for more details.

Whilst we are one practice, providing a range of services across three sites (See Practice Area Map), all patients registered with the practice can access services from any of the three sites, and see a healthcare professional of their choice, regardless of where they live.
