New Patients

To register as a new patient please contact our reception team and request a new patient registration pack. Once completed, please return it to reception with the relevant identification. The form will then be passed to our Administration Team who will arrange for you to be added to the practice register. All new patients will be invited to attend a new patient health check. Medical treatment is available from the date of registration.

PLEASE NOTE you will also be asked to produce photo ID (passport, drivers licence etc.) and proof of address to validate your registration.

From the 5th January 2015 we are able to offer to patients out of our natural catchment area the opportunity to register with us as an 'Out Of Area' patient which means they can still attend the practice for appointments etc but are not eligible for home visits. Ask at reception for more details.

Whilst we are one practice, providing a range of services across three sites (See Practice Area Map), all patients registered with the practice can access services from any of the three sites, and see a healthcare professional of their choice, regardless of where they live.

Practice Area Map

The practice accepts new patients from Hemsworth, Kinsley, Fitzwilliam, South Kirkby, South Hiendley, Upton and South Elmsall.
