New Patients

  • Register with the Practice

To register as a new patient please either use our online registration form or  contact our reception team and request a new patient registration pack. Once completed, please return it to reception with the relevant identification. The form will then be passed to our Administration Team who will arrange for you to be added to the practice register. All new patients will be invited to attend a new patient health check. Medical treatment is available from the date of registration.

Practice area

Whilst we are one practice, providing a range of services across three sites (See Practice Area Map), all patients registered with the practice can access services from any of the three sites, and see a healthcare professional of their choice, regardless of where they live.

The practice accepts new patients from Hemsworth, Kinsley, Fitzwilliam, South Kirkby, South Hiendley, Upton and South Elmsall. You may find it helpful to use the 'postcode checker' below as this will indicate whether we accepting patients from your area. To register as a patient please speak to reception.
