In line with our legal duty to protect patient confidentiality, we require a patient’s explicit consent before we can speak to, or take instructions from, anyone else on their behalf.
This applies even if the request is only to book or change an appointment and includes:
- Family members
- Friends
- Carers
- Support workers
If consent has not been recorded, our team will not be able to discuss or arrange appointments or share any information about the patient.
How to give consent
- Patients can provide consent directly to the practice, or
- Consent can be given in writing (please use the consent form below)
